To Request SASO DVDs—PRIVATE SCHOOLS
NOTE 1: All middle and intermediate public schools in WV were contacted and asked to provide the name of the appropriate person to received the DVDs which were sent the week of September 24, 2007. Only one DVD set (6 DVDs) was sent to each school. Please contact your principal, secretary, or the faculty member who is responsible for character education in your school.
NOTE 2: Sending the DVDs to high schools is not permitted because doing so constitutes an NCAA recruiting violation.
NOTE 3: Private middle schools in West Virginia may request the DVDs if they complete this form to document that the highest grade level in the school is the 8th grade.
